Job Description:
A company is looking for a Nurse Practitioner to perform in-home
health assessments on Medicare Advantage members.
Key Responsibilities
Conduct in-home health assessments for Medicare Advantage members
within designated counties
Complete annual health assessments, each lasting 45-90 minutes
Work flexible hours, including weekends, based on personal
availability
Required Qualifications
Minimum of 6 months experience as a Nurse Practitioner
Active NP license and national certification (ANCC or AANP)
All board certifications accepted except PMHNP or WMNP
Experience in gerontology, cardiology, internal medicine, or
endocrinology is a plus
Home health care or home visit experience is a plus
